Athletic Forms
The following information goes over the documentation required for students to participate in athletics within Rio Rancho Public Schools.
Forms and Physical Submissions
Step 1: Download and print the NMAA Physical Packet and give to your Physician to complete.
Step 2: Complete the NFHS Student Concussion Course For Students. Once completed download the certificate and submit with the following documents. Please note only certificates with your students name on it will be accepted.
Step 3: Copy the front and back of your health insurance card. If you do not have health insurance and need to file a claim please review the following information below:
Step 4: Please submit all forms through the .Athletics Documents Upload Form System Once all forms have been submitted and reviewed for accuracy you will be notified via email that your student is cleared for athletics. If your documents are not complete you will be notified via email.
