Dear Parent or Guardian:

The School Wear Assistance Form is intended to help the school district identify families and children who are unable to afford the expense of clothing that fits the school district's School Wear Guidelines. The school district is working in partnership with Storehouse West, a Rio Rancho-based non-profit organization that provides short-term assistance to the community, in providing School Wear Assistance. Qualifying income levels are listed on page 2 of the form.

Storehouse West will begin reviewing applications starting July 14, 2008.

Storehouse West is located at 1030-F Veranda Drive, Rio Rancho, N.M. 87124 (south of Southern Blvd., two blocks west of Unser Blvd.). Applications are accepted Mondays and Fridays from 9:00 am to 12:00 noon and on Wednesdays from 12:00 noon to 3:00 pm.

Since the school board voted to reinstate denim blue jeans as part of the dress code, the district will no longer provide assistance for pants. The School Wear Assistance Program will now consist of supplying three polo shirts per student to qualifying families. Storehouse West will now be our distributor for polo shirts. The district will provide navy blue shirts in children and adult sizes.

The School Wear Assistance Form must be completed and all supporting documents must be presented at the time of your appointment. Documents you must supply include:

• photo ID
• proof of address with current date (PNM bill, lease, etc.)
• birth certificates or baptismal certificates
• social security cards
• paycheck stubs for at least two consecutive pay periods or TANF letter, or
• proof of unemployment and/or benefits, or
• proof of Social Security benefits
• proof of additional dependents, etc.
• if applicable, proof of divorce

We will make copies of all supporting documents for our files. All information supplied to the school district and Storehouse West will remain confidential.
