Below are the forms you will need to print, then complete and return to the school in order for your child to participate in athletics in grades 7 to 12. Be sure you have the Adobe © Reader to open these forms (download here if necessary).

Here are the forms you will need (depending on grade level). If required, just print, complete, sign and bring in:

Athletic Handbook: All Grades (PDF format- 42 pages)
Student Athletic Code of Conduct: Athlete behavior/consequences and new drug screening information
Athletic Participation Contract: All Grades
PRE/POST Event Parent Transportation Request: To request permission to transport your child to/from athletic events
Sports Physical: All Grades
Emergency Card: All Grades
Authorization to Administer Medication at School (non-asthma)
Athlete Information: 7th Grade
Athlete Information: 8-12th Grade
Website/Media Release Permission: (this is a permission form; please read carefully before signing)
