Licensed Personnel Application
Please follow these instructions for completing, printing, and submitting our online Licensed Personnel Application very carefully. (Please note that a new online application process will be implemented in September 2008...read the details here.)

Important Application Instructions:
In rare cases, the printed application shows inverted or unintelligible text when printed using the browser. This is due to the browser printer engine attempting to print a PDF form.
The solution is to open it in Adobe Reader:
1. Open the application in your browser as usual.
2. Save a copy of the application to your desktop by clicking the "Save" button on page 3 of the application.
3. This will save a copy of the application on your computer.
4. Then just open your local copy (it will open in Adobe
Reader, not the internet browser).
5. Fill it out accordingly, then PRINT (this will use Adobe's
printer, not the browser's, and you won't see the corrupted
text).
Please don't hesitate to contact the Internet Services Manager contact the Internet Services Manager if you're having trouble getting the application to print
correctly.

HOW TO APPLY:
- Complete a Transcript Analysis using this form: Transcript Analysis (Get instructions here).
NOTE: this is an online form meaning you can click in the boxes and just type! The hours will total automatically for you at the bottom. For a list of acceptable courses in each content area, look on the Instructions for Completing Transcript Analysis.
Complete this form online, click to Save, then Print the form (requires the latest version of Adobe Reader).
- Complete the Licensed Application form below (pointing hand). You'll add your signatures after you print it.
Steps to complete:
- complete all the boxes that apply to you,
- click the Save button (requires the latest version of Adobe Reader) to save a completed application (page 3),
- click the Print button
- add your signatures
- finally, mail the form to us.
- Make a copy of your college transcripts and any other supportive documents you wish to submit with your application.
To help you complete the (pdf) form, here are a few tips:
- Use your TAB button to move forward from box to box.
Use SHIFT + TAB to move backwards through the boxes. - If you need to, use the CLEAR button (page 3) to clear all the fields and start again; otherwise just select text you need to change and retype
- MAKE SURE all the boxes are filled in that apply.
- Dates will need 4-numbered years (ex.: 1985).
- Use an extra sheet if you need to add information, and send it with your application.
- When the entire application form is complete, preview it to make sure you have not left any necessary fields incomplete.
- Fields that you don't complete will remain blank.
